The program enables individuals with a permanent or chronic disability to live in their own residence or community with needed support by their Personal Care Attendant.

Eligible individuals are MassHealth recipients and need physical assistance in two areas of Activities of Daily Living skills (ADLs).

ADLs are:

  1. Mobility

  2. Assisting with medications

  3. Bathing or grooming

  4. Dressing or undressing

  5. Passive range of motion exercise

  6. Eating

  7. Toileting


To be eligible for this program, an individual must:

  1. Be a MassHealth recipient

  2. Have a permanent or chronic disability, which impairs functional ability to perform at least two activities of daily living without physical assistance.

Application/Approval Process

The applicant and a surrogate, if necessary, will participate in an initial assessment meeting, which determines the individual’s functional capabilities and need for assistance.

Following this assessment, an Evaluation, conducted by a Registered Nurse and Occupational Therapist, will be scheduled and completed in the applicant’s home.

The Application and Evaluation signed by member’s primary care physician are then submitted to MassHealth for approval.

If approved, the Arc’s PCM Staff will provide assistance to begin and to maintain services throughout the first and subsequent years for each recipient.
