Position: Part Time Adult Foster Care (AFC) Manager

Summary: Part time Care Manager position (approximately 10 hrs. a week) for a positive, friendly, and caring individual for our Adult Foster Care (AFC) program. The Care Manager regularly visits individuals and their families at their homes in and around the South Shore area. The visits take place once a month, or more often as needed. The Care Manager provides oversight to ensure the living situation is safe, healthy, and cohesive for all parties involved. The Care Manager also ensures the families and individuals are in compliance with Mass Health AFC regulations. Hours are flexible and may include some weekends and/or early evenings.

Position requirements:

  • Bachelor’s Degree required. Social work background preferred.
  • Minimum 5 years experience working with the elderly and/or individuals with developmental disabilities.
  • Valid driver’s license and reliable transportation a must.
  • Acceptable Driver history, CORI, and SORI results
  • Fingerprinting may be required

Key responsibilities:

  • Conduct assessments and obtain social histories on AFC members and Caregivers.
  • Support, train, evaluate, and supervise the AFC Caregiver in conjunction with the RN.
  • Conduct on site home visits and complete Care Manager progress note and other paperwork as required.
  • Ensure all documentation and forms are complete, accurate, and timely.
  • Excellent interpersonal, organizational, and communication skills.
  • Ability to independently manage a caseload and provide all encompassing care management.
  • Excellent documentation/writing skills.
  • Ability to think and work creatively as part of a team and/or independently.
  • Ability to manage challenging behaviors.
  • Perform other tasks as needed or assigned

Physical environment and demands: While performing duties for this position, the employee travels by using their own personal vehicle and is exposed to changing weather conditions. The employee must have the ability to safely travel to and from the AFC home. In addition, the employee will be required to drive for meetings and/or visits to AFC provider homes or to facilities targeted for AFC program marketing. Travel may be extensive so there will be a lot of sitting in an automobile. The employee will be required to work in and safely navigate varied facility/residential/home environments which may include interior and/or exterior stairs or elevators. While at the consumers’ home, the employee may be required to sit, stand, lift and carry files, laptop and associated equipment, listen attentively, talk, write/record, explain and gather information. Since AFC provider homes are private residences, the employee may be exposed to pets, family members, smoking, cooking or other fragrances. The noise level in the work environment is moderate. Moderate levels of stress may occur.

Position: Fee Based RN-Adult Foster Care

Summary: Fee based (under 20 hours weekly) position for a positive, friendly, and caring RN. This position is for our Adult Foster Care program. The RN regularly visits individuals and their families in and around the south shore area on a monthly basis. The RN provides oversight to ensure the living situation is safe, healthy, and cohesive for all parties involved. Hours are flexible and may include some weekends and/or early evenings.

Position Requirements:

  • Current Massachusetts RN nursing license
  • Bachelor’s Degree preferred
  • Minimum 2 years recent experience with individuals with developmental disabilities, and/or the elderly, and/or medically involved individuals
  • Valid driver’s license and reliable transportation a must.
  • Acceptable Driver History, CORI and SORI results required
  • Fingerprinting may be required

Key responsibilities:

  • Completion of initial and annual assessment forms (MDS), and admission paperwork
  • Develop a plan of care for each member and keep updated
  • Support, train, and evaluate caregivers
  • Provide oversight for all medical needs
  • Schedule home visits, in conjunction with the Care Manager and families.
  • Document home visits and any communication with caregivers and members.
  • Communicate with health care providers as needed
  • Perform other tasks as needed or assigned

Summary: RESIDENTIAL COUNSELORS – Relief
Relief staff needed! Must have 1st Aid Cert., valid driver license, acceptable CORI & clean driver record. Work hands-on w/MR population.  Responsibilities include ensuring resident health & safety, developing & implementing ISP goals & recreational & community supports. Experience in field of developmental disabilities preferred. MAP Certification a plus!
